Specialist Support Coordination

Specialist Support Coordination is provided for participants with complex support needs or high levels of risks. The service is provided by a qualified and experienced practitioner to support participants to address complex issues. Together, we focus on reducing barriers in the environment, connecting with broader systems of support, and meeting the individual needs of each participant.

MPSW provides Specialist Support Coordination, including support to consult and negotiate with multiple stakeholders to come up with the best possible support solutions, achieve well-coordinated plan implementation to achieve individual goals, manage crisis and assist stakeholders with resolving points of crisis for the participants. Depending on individual participant’s circumstances, MPSW may also design a comprehensive care plan that involves all stakeholders in a participant’s life. The plan would support the stakeholders to work together towards resolving obstacles they face and enabling smooth service delivery to promote the participant’s wellbeing.

Coordination of Supports

Coordination of Supports is provided for participants to strengthen their ability to design and build their supports across mainstream, community, informal, and funded services. Participants are supported to build and maintain a resilient network of formal and informal supports, and to direct their own lives more confidently. Participants are also supported to understand their plan and funding, identify their expectations from services and how they want their supports to be designed. This is achieved through collaboration and consultations.

Psychosocial Recovery Coaching

Psychosocial Recovery Coaches provide assistance for participants to build capacity and resilience through strong and respectful relationships. The assistance is to support people with psychosocial disability to create and live a full, meaningful, and contributing life as they wish. We work collaboratively with participants, families, carers, and other services to identify, plan, design, and coordinate NDIS supports in the complex system and maintain engagement.

Psychosocial Recovery Coaching includes, but is not limited to –

  • Developing recovery-enabling relationships
  • Supporting participants with their recovery planning
  • Supporting engagement with the NDIS, including support with plan implementation
  • Coaching to increase recovery skills and personal capacity, including motivation, strengths, resilience, and decision-making
  • Collaborating with the broader system of supports to ensure supports are recovery-oriented
  • Documentation and reporting. (Source: NDIS)

Therapeutic Supports / Social Work Supports

Therapeutic Supports / Social Work Supports are provided for participants in various situations and with a range of issues and support needs.

Participants are supported to build their capacity through specialist assessment of skills, abilities, and needs; holistic assessment of participants’ environment including protective and risk factors; therapeutic supports and counselling, coaching, mentoring; and/or assistance:

In the areas of:
– various individual and social issues
– development of independent living, life skills, and resilience
– life stage planning and transition, such as in accommodation, employment, and education

This is an individualised support for participants to –

  • address complex or unclear needs; complex personal, inter-personal and social
    issues; and barriers in their unique circumstances.
  • focus on recovery and capacity building using their strength.
  • gain greater stability, insight, self-knowledge, emotional acceptance and
    growth, and the optimal development of personal resources, to work towards
    their personal goals.
  • improve social interactions and connect with other people and broader
    communities and systems of support
  • manage and adjust to the participant’s changing needs
  • enhance the quality of life and wellbeing of participants and their
    significant others
  • exercise autonomy, self-determination, and self-directed life
